• Ship this completed rebate form, a copy of the invoice for your eligible Cisco SMB product

purchase, and the qualifying Linksys product(s) (original packaging not required) to:

Be sure to retain copies of your invoice and this rebate form for your records.

Although you may only trade up one Linksys product to one Cisco product, you may use this form

to process your rebate for up to five eligible Linksys products for five eligible Cisco products. Your

Upon receipt and review of your shipment, you will receive an e-mail confirmation stating your

eligibility and the rebate amount to be issued. You should receive your rebate check payable to

your organization within four to six weeks of the confirmation date.
